Clinical Liaison Responsibilities:
Provide an interface between health care providers and Traditions Home
Health Services.Develops and implements a program to contact all potential referral sources
within the hospital.Responsible for establishing, growing, and maintaining referral volume and
relationships with hospital staff.Communicates with existing referral sources and identifies new
opportunities within the hospitalDelivers exceptional customer service, and assists with advanced care
planning discussions, and informative private home care consults for clients
and family membersResponsible for ensuring the intake and referral processes are performed
effectively and timely to meet the needs of our clients, families and referral
sourcesCompletes the admissions process on referred clients in emergency cases, as
staffing indicates, and for inpatient private home care admissions.Provides accurate and timely information regarding hospice services to
anyone inquiringReceives calls from referral sources, responds timely, and initiates
paperwork to begin Intake/referral process.Serves as client advocate in evaluating & developing plan of care.Offering in-services and education on the benefits of Private Home Care
Services.Provides an interface between health care providers and Traditions Home
Health Services.
