The Nurse Case Manager is responsible for providing high quality care in the community and outpatient settings. The Nurse Case Manager is expected to function well as a member of a multidisciplinary Clinical Team. The Nurse Case Manager is responsible for utilizing comprehensive skills in assessment, treatment planning, care coordination, case management, medication administration, crisis intervention and the documentation of services provided. The Nurse Case Manager will also provide training and instruction to staff to ensure that the highest quality of care is provided to the consumers in such areas as, but not limited to, AMAP training, First Aid/CPR, Bloodborne Pathogens, and OSHA requirements (Tuberculosis Awareness instruction and annual screening). All services provided are to promote client independence of daily functioning.
