Some Job Responsibilities:
Development and execution of strategic clinical initiatives aimed at improving the quality of care and optimizing patient outcomes across all home care services.
Collaborate with senior leadership to establish clinical priorities, ensure alignment of clinical operations with business objectives, and identify opportunities for enhanced efficiencies.
Work collaboratively to address priorities within the CHHA, such as reducing hospitalizations, value-based purchasing, care coordination, and patient education.
Oversee the design, implementation, and evaluation of clinical programs and initiatives in certified home health programs.
Ensure that all clinical initiatives are aligned with industry best practices, regulatory standards, and organizational goals.
Develop, implement, and continuously assess care protocols, guidelines, and clinical policies to ensure high-quality, safe, and effective home care services.
Lead quality initiatives to improve the clinical outcomes, patient satisfaction, and overall effectiveness of home care services.
Monitor clinical performance through data analytics, patient feedback, and other quality measures to identify trends.
Ensure compliance with all New York State and federal regulations, including those set by the New York State Department of Health (DOH) and Centers for Medicare & Medicaid Services (CMS).
Work collaboratively to prepare for and respond to regulatory audits and surveys.
Oversee the clinical management team to ensure the effective implementation of clinical initiatives and best practices.
Serve as a resource by offering expert advice on clinical protocols, care delivery, and quality improvement strategies.
Collaborate with clinical leadership to ensure consistent, high-quality care is delivered in alignment with organizational goals and regulatory standards.
Ensure collaboration between interdisciplinary team members, including case managers, social workers, home health aides, and other healthcare professionals, to ensure coordinated and comprehensive patient care.
Cultivate and maintain relationships with external stakeholders such as healthcare providers, referral sources, payers, regulatory bodies, and community organizations.
Utilize data analytics to monitor clinical outcomes, track performance metrics, and identify areas for improvement.
Use data insights to drive continuous improvement efforts and support evidence-based clinical decision-making.
Prepare and present reports on clinical performance, current projects, and outcome metrics.
Ensures that all activities performed align with the mission and vision of Visiting Nurse Service & Hospice of Suffolk, Inc.'s board of directors, executive team, and the leadership team.
